Booking Quality Transportation Has Never Been So Easy

If you’re interested in working with Livin’ It Up Transportation and booking one of our vehicles, the process is quite simple. All you have to do is download the contract below, fill it out completely, and send it back to us. Once we receive the contract, we’ll contact you to finalize your transaction, answer all of your questions, and make all of the final preparations needed to ensure that you and your friends have an incredible experience.

Livin’ It Up Transportation will provide a sober driver to escort you on your party adventure. We do not encourage drinking,
but if there is drinking, we want to make sure you have a safe and sober way home. We want to stop people from drunk
driving and let everyone know we are their safe ride alternative.

To ensure that everyone will have a great time and a safe trip, we would like every guest to abide by our safety rules listed
below:

ALL GUESTS ON THE BUS MUST BE 21 OR OLDER TO CONSUME ALCOHOL ON THE BUS.

Livin’ It Up Transportation is not responsible for any personal items on the bus during or after the trip. Any items left on the
bus can be picked up by calling Adam to make arrangements. 217-821-9083

All cancellations are required to be made at least 72 hours in advance. Any cancellations within 72 hours of departure will
result in the customer paying for the entire day/evening.

NOTE: To guarantee your reservation, you must fill out the form completely, and return the contract. A credit card deposit in
the amount of $150 will be due upon booking the bus. All deposits are Non-Refundable. The final balance payment will be due
on the day/evening of the event before the bus will depart from the pick-up location. Only cash will be accepted for the final
payment.

Any lost, broken, or stolen equipment will result in a replacement fee. The bus must be left in the same condition as you
began. If damage is found, additional charges may be applied after the event. These damages will be documented and mailed
to you.

If trash is not disposed of properly in provided trash cans or a passenger get sick on the bus, it will result in a $75.00 cleaning
fee charged to your credit card. Livin’ It Up Transportation will provide cleaning supplies and disinfectant to clean up any mess.

No smoking, illegal drugs, underage drinking, firearms or fighting are allowed. ANY VIOLATIONS OF THESE OR ANY OTHER
ACTIONS DEEMED UNACCEPTABLE MAY RESULT IN THE IMMEDIATE CONCLUSION OF EVENT WITHOUT A REFUND. Contract
signer is responsible for all event actions on the bus.

Livin’ It Up Transportation agrees to send the requested vehicles as offered in the contract. We have the right to switch the
vehicles in case of emergency breakdown or if the vehicle is damaged. A replacement vehicle may be substituted if the
contracted vehicle becomes unavailable for any reason before or during the trip. Customers therefore accept the replacment
vehicle and will NOT receive a refund.

Livin’ It Up Transportation is not responsible for any falls, accidents, or mishaps on the bus.

Additional Hours may be purchased for $________ per hour. Amount will be due in cash during your event.
